Tonsil and adenoid surgery

Tonsils and adenoids need treatment when they are infected often or grow large enough to narrow the airway. Whether to operate depends on how often and how much the problem affects you.

What are tonsils and adenoids?

The tonsils sit on both sides of the throat and the adenoids at the back of the nose. Both are lymph tissue and can grow and become infected often, especially in childhood.

When is surgery considered?

  • Seven or more episodes of tonsillitis in a year, or five or more a year for two years in a row
  • Snoring, pauses in breathing during sleep and mouth breathing
  • Repeated middle ear infections or ear fluid caused by the adenoids
  • An abscess around the tonsil
  • One-sided tonsil enlargement without a clear cause

Assessment

The size of the tonsils is checked at the examination; the adenoids are seen with an endoscope or, if needed, an X-ray. In children, pauses in breathing at night and the state of the ears are also asked about. Hearing and ear pressure tests are done if needed.

The operation

The operation is done through the mouth under general anaesthesia with no external incision. Tonsils and adenoids are removed together or separately as needed. Adenoid surgery usually takes 20–30 minutes and tonsil surgery 30–45 minutes.

Recovery

WHENWHAT TO EXPECT
Day 1Most people go home the same day or the next day.
Week 1A sore throat is normal; plenty of fluids and soft food are advised.
Days 5–10Scabs come away in this period; seek help at once if there is bleeding.
Week 2Most children return to school and adults to work.

Times are averages and vary from person to person.

Frequently asked questions

Does removing the tonsils weaken immunity?

No. The body has plenty of other lymph tissue. Studies show tonsil surgery does not harm immunity.

Can the adenoids grow back?

Rarely, especially if they were removed at a very young age.

What should I eat after surgery?

Cold and lukewarm drinks, yoghurt and puddings are recommended in the first days. Avoid hard, very hot and spicy food.

Is tonsil surgery done in adults too?

Yes, for adults with frequent infections, abscesses or breathing problems during sleep. Recovery can take a little longer in adults.
